Nurse Practitioner

Nurse Practitioner:

Partner with like-minded health professionals to address preventive, chronic and acute care needs. You'll be part of a collaborative team that integrates a whole health coach focused on behavior change, and together will support patients in reaching wellness goals.

Whole Health Coach

Whole Health Coach:

Integrated as a key member of the clinical care team, you'll work collaboratively with diverse patient populations to engage them in their well-being and support them in achieving personal health goals. You'll facilitate coaching across individuals and groups, primarily in-person.

LPN/LVN or Medical Assistant

LPN/LVN or Medical Assistant:

Own a diverse range of administrative and clinical tasks to assist our clinical providers. Deliver excellent patient service and ensure the clinic runs efficiently by managing front office duties and patient flow.
